New Student Employee

As a new student employee, the following steps explain important information required to be completed in order for you to start working and getting paid. Please note you will NOT get paid if you do not complete the following paperwork in Step 1. Take a few moments and read through the summary and corresponding attachments. If you have any questions, feel free to reach out to payroll@northpark.edu or humanresources@northpark.edu.

STEP 1: Student New-Hire Personnel Paperwork
This is a very important step in the process as we will not be able to set you up in payroll or get you paid until all of these forms are completed in this step. Once you have completed the forms, please digitally submit them to payroll@northpark.edu. Once you have emailed these completed forms, you will need to stop by the HR office to present the proper identification to the HR department. We cannot process your new-hire paperwork without this step. The most common identification(s) provided are a current Passport OR current Driver's License AND social security card or birth certificate. The identifications shown must be originals and not photocopies.

Please complete the following forms and submit them via email to payroll@northpark.edu (only digital copies are being accepted at this time):

  1. Student Data Sheet

  2. Direct Deposit Form (All payroll is paid through Direct Deposit. If you do not have a bank account, a variety of free online services are available, all of which will provide a routing number and bank account number for Direct Deposit. Some examples are: Ally, CashApp, Chime, Discover, GreenDot, PayPal, and Venmo).

  3. Federal W4 Form

  4. Illinois W4 Form

  5. Illinois W5 Form (Only for residents of IA, KY, MI & WI)

  6. I9 Employment Eligibility Form
